We focus on all types of orthopedic problems and pelvic floor problems. She has over 23 years experience in outpatient orthopedics, hospitals, in-home services, skilled nursing settings and occupational health. She has a passion for quality, patient centered, outcome based Physical Therapy. She believes in patient education and a balance of targeted exercises, manual therapy and home exercise programs for each patient to reach their goals. After working for 20 years in the rehab industry in many different setting Heidi decided to open Community Care to provide a more patient focused treatment model.

Her journey began at the IFMK of Vichy, where she graduated in 2001 with a French National Diploma in PT. In 2009, Delphine relocated to San Diego. Her international experience and diverse skill set have enriched her practice, allowing her to provide comprehensive care to a wide range of patients.While in France, Delphine worked extensively with the post-pardum population, gaining valuable expertise in this area. Focused on continuing education, she furthered her specialization in pelvic health through the renowned Herman and Wallace Institute. She firmly believes in tailoring her approach to each patient, ensuring they reach their full potential and improved health and well-being.

Lexie's continuing education is focused on rehab and health of pelvic floor through the Herman and Wallace institute to gently guide clients through success with goals related to voiding function, intimacy and returning to movement joyfully! Marcy will provide you with warm friendly service at our Liberty Station office. She works hard to help you find a schedule that works for you, verify your insurance coverage and answer any of your questions. Marcy can assist you in English or Spanish.
Our rehab tech team at both locations work in both our front office and as a rehab techs. They will greet you warmly in person or over the phone. Our rehab tech team is multilingual including Spanish and Tagalog. They will make sure your questions are answered and your needs are met.

Community Care Physical Therapy's is now accepting the following insurances:
No, you do not need a MD's referral to go to physical therapy. California has direct access to PT. You may visit your PT for 12 visits or 45 days without a referral.
Summer 2024 $75 SPECIAL: ALL (including initial evaluation) self pay visits $75 through 9/30/24 (ortho).
Visa/Mastercard/Discover/FSA/HSA cards accepted. (please check your individual plan)
Private Pay Initial Evaluation is $100. (45-60 minutes)
Private pay follow visits are $85 (45-60 minutes)
Expect to be greeted by the friendly staff at Community Care. Your visit will be about 45-60 minutes long. You will fill out some initial paperwork (emailed to you prior to visit) so that the therapist has a better understanding of your injury, current health status, functional level and goals. Your PT will evaluated the injury, discuss a plan, provide treatment and assign you a home exercise program if appropriate.
